Starting this October…
ACHS Offers FREE Diabetes Self-Management Program
Ammonoosuc Community Health Services, Inc. is pleased to announce a FREE diabetes self-management workshop starting this October at the Saint Rose of Lima Church, in Littleton. The Stanford University, evidence-based, Diabetes Self-Management Program (DSMP) will run on Wednesdays, from October 17thth to November 21st from 1:30 pm-4:00 pm. This workshop is a 6-week program and offered FREE to community members who have type II diabetes, prediabetes or who might be caregivers of someone with diabetes. DSMP workshops are offered in a fun, group setting and focused on discussions and sharing practical suggestions for managing diabetes. You’ll discover:
- How to eat healthier
- Why setting weekly goals are so important
- Which physical actives can help
- Techniques for improving communication with your health care provider
- Plus, much, much more!
