As a Patient-Centered Medical Home (PCMH), we prioritize outcomes and integrated care, providing primary care, behavioral health, and chronic disease management to almost 10,000 patients across 26 towns in southern Coos County and northern Grafton counties. Our five sites – Littleton, Franconia, Warren, Whitefield, and Woodsville – are all dedicated to providing exceptional care.

Located in the stunning White Mountain National Forest area of New Hampshire, ACHS offers incredible natural beauty and endless opportunities for outdoor activities. The slower pace of life in this rural area supports a healthy work-life balance, but its location is still just 2.5 hours from Boston, and even closer to Montreal, Burlington, VT, and the coast of Maine.

  • CMA / RMA / LPN - (Multiple Positions)

    POSITION SUMMARY: The CMA / RMA / LPN position is primarily responsible for assisting providers in delivering quality health care to ACHS patients. The ideal candidate will have the ability to provide superb customer service and quality care. Candidate must be a team player and play an active role in the core clinical team. We currently have multiple positions at our ACHS sites.

    Skills & Requirements
    • Must possess excellent customer service skills.
    • Must be able to deal with the public, often in stressful situations.
    • Excellent spoken and written communication skills.
    • Good computer skills a must.
    • Ability to maintain strict patient confidentiality.
    • Work independently with a minimum amount of supervision.
    • Medical office experience preferred.

    Additional Abilities

    • Obtain and record patient information such as vital signs, patient history, and prescription history.
    • Perform diagnostic lab work.
    • Administer oral medications and immunizations.
    • Maintenance of patient data and health information in the electronic health record.
    • Other duties, hours, and ACHS site placement as assigned.

  • Patient Access Specialists
    POSITION SUMMARY: Do you LOVE people? Understand the importance of friendly, kind, customer care? Have empathy and caring nature? If so, ACHS is recruiting for an experienced Patient Access Specialist for our Primary Care and Behavioral Health locations.

    The Patient Access Specialist facilitates the smooth transition of patient care and provides a wide range of critical tasks that support an efficient, effective, and financially sustainable health care center.  As the initial voice and face of our health care facilities, the Patient Access Specialist assumes responsibility for enhancing the overall ACHS patient experience. The successful candidate MUST enjoy meeting and greeting the public. The Patient Access Specialist may also cross-train in our dental and oral health center; ensuring a continuous learning environment.

    Skills and Requirements:

    • Must possess strong computer skills and be proficient in the MS Office software suite.
    • Must be able to communicate effectively with others using written and spoken word.
    • Ability to perform work accurately and thoroughly.
    • Excellent organizational skills and ability to follow SOP’s.
    • Must enjoy the public and present excellent customer-relation skills.

  • Family Practice Providers

    We’re Putting “CARE” back in healthcare

    ACHS has a culture of care that is different than other organizations. Here we offer truly integrated care, where primary care, dental, behavioral health and support services work together to wrap around our patients and deliver the best in quality care.  This environment fosters innovation and team participation and leads to strong, fulfilling careers for our family practice providers – many who have been with ACHS for decades. We’re searching for like-minded, energetic, mission-focused Family Practice Providers to join our dedicated team to accommodate growth and additional access to health care for our 11,000 patients. ACHS currently has 8 Family Practice Physicians, 7 APRNs, 4 Physician Assistants, 1 Pediatrician, 1 Internal Medicine Physician, 1 Pharmacist, 3 Dentists, 5 Behavioral Health specialists attended by a robust team of clinical and non-clinical support personnel.

    ACHS Physicians will experience:

    • A balanced focus on quality & volume allowing more time with patients: average 20+ min visits.
    • IT support team with a strong, clinical knowledge with >20 years of EHR experience.
    • Integrated clinical pharmacy, dental and oral health services, behavioral health and patient navigator support, nutrition and paralegal services.
    • Emphasis on self-care for patients and staff: provider driven autonomy in practice.
    • Varied patient mix – pre-natal through the end of life.
    • Compensation is between 75%-90% of MGMA, NACHC, and Medscape scale.
    • May be eligible for retention bonus or relocation assistance.
    • Extensive growth opportunities in clinical areas of interest and leadership.
    • An innovative leader in second round of CMS ACO Pilot Projects.
