Ammonoosuc Community Health Services is pleased to welcome Amy Page, MS, APRN, FNP-C to its team of Family Practice providers. Amy is welcoming new patients and providing care to the existing patients of ACHS now that Evelyn Hagan, APRN has retired. Amy specializes in Women’s Health, Chronic Disease Management, Pediatrics and Geriatrics and will have clinic hours at the ACHS-Whitefield care delivery site.

Amy graduated from the University of New Hampshire. Earning a Bachelor of Science degree in 2012 and a Master of Science in 2016. She is certified by the American Academy of Nurse Practitioners (AANP) as a Family Nurse Practitioner. Amy completed her internship at Coos County Family Health Services and Weeks Medical Center. ACHS COO, Teresa Brooks said recently, “Amy is friendly and outgoing, and truly cares about her patients. She provides the same warm, caring attention they have come to enjoy at the Whitefield site.” Amy is looking forward to building positive partnerships with her patients and is grateful to be part of a Community Health Care organization where comprehensive, affordable, high-quality care is available to all, and helps to fosters wellbeing in the community.
